Athletic Health Forms

The Spring Sports season begins on Monday, November 17th. A sports physical must be completed before your child can participate.

If your child has had a sports physical earlier this year, Parents and students need to complete the the Recertification Section 5 form. Click here to download the Section 5 form.

If there are any “YES” answers in the supplemental health section, please download and complete Section 6, which will require a doctor’s signature. Click here to download the Section 6 form.

PIAA has made it clear that if your child has had a sports physical with your own doctor, any physician can sign the Re- Certification form as long as you have the original physical available for the physician to review. Therefore, if you DO NOT have the original physical, you need to see the original doctor.

If your child has played a Fall or Winter sport and has been seen by your own doctor and wants to get re-certified by the physicians provided by the school; their original physicals will be available for them to take to the school’s physician.

At the Middle School, the form(s) is available from Mr. Hassler, Mrs. Sheetz or the main office. At the Upper School , the form(s) is available from Ms. Figueroa or the main office.

If your child has not had a sports physical this school year, they can get a free physical at the school or schedule one with their own physician. Moravian Academy offers free sports physicals to all athletes for each sports season. The doctors will be on campus Wednesday, November 12^th. The doctors will be providing a sports physical screening, not a full physical; therefore, testicular exams will not be provided.

Middle School students must complete two forms prior to having the physical.

The first form is for Middle School parents and students to complete. In Section 2, please sign across from all sports your child may participate for the entire year. Parent/Student Forms

The second form is for the physician to complete. You only need to download this form if you are using your own doctor to complete the physical. Doctor Evaluation Form

Upper School students must complete must complete two forms prior to having the physical.

The first form is for Upper School parents and students to complete. Parent/Student/ImPACTForm This form includes a parent consent form for the ImPACT concussion management program that we are using with all Upper School athletes. This program will enable us to better evaluate athletes who have suffered a concussion. You can visit the ImPACT web page at www.impacttest.com to get more information about the software. In Section 2, please sign across from all sports your child may participate for the entire year.
